Maryland Treatment Centers is committed to protecting your privacy. To request your or your loved one’s medical records, please complete the secure HIPAA-compliant form below. We encourage electronic submission through this form for faster processing.

 

If you prefer to complete the request manually, you may download and print the ROI form here (PDF) and fax it to us at 410-233-0184 or mail it to the address listed on the form.

Please Note:

We’re here to help you access the information you need.​  In accordance with state and federal guidelines, we retain medical records for 12 years from the date of your last visit.  Maryland law allows up to 21 business days to process and respond to your request.

If you need your records by a specific date, please let us know. While we will do our best to meet your timeline, we cannot guarantee early completion.
